Care When Leaving Hospital

Each year approximately 10 million people are admitted to hospital wards and of those, half a million people will be re-admitted within 30 days of receiving hospital treatment.

One of the main reasons for patients having to return so soon after being discharged is thought to be that their actual discharge was too early.

In addition, for many people, especially our older population, there is insufficient care when leaving hospital in place when they return home. There may be care from the statutory sector, from friends and relations but sometimes there is no care available and many times, the care cannot be provided for a long enough time period.

For any person being re-admitted to hospital, it is a demoralising time and places extra strain on the families and friends of the patient.

Many families, especially those who live some distance away and who are unable to give the time and care they would like, turn to the solution of having a live-in care package to ensure that a loved one is enabled to re-habilitate at home safely and without risk.

Live-in carers will provide the domestic care a home needs, help with personal care such as helping to shower and dress a person, prepare meals, shop, launder and provide companionship at a time when most peoples lives socially are reduced as they try to get back on their independent pathway.

Able Community Care has been providing live-in care packages to people since 1980 and our services are available in England, Scotland, Wales and The Channel Islands and similar live-in care services can be provided where possible, outside of the UK.

If someone you know would like extra help and care after leaving hospital to return to living independently, please ask for our current brochure by calling 01603 764567, email to ablemg@aol.com or click here and submit your request online.
