Many people are discharged from hospital too early and return home without appropriate care arrangements being in place.
Figures show that as a result many older people are re-admitted within weeks of their discharge as an emergency admission.
For many decades convalescent homes provided the 'in between' care enabling patients to go home with the independence needed to resume their lives. This transition period between hospital and home would build up independence, aided recuperation and would enable care staff to monitor that all was well thereby enabling the 'patient', after one or two weeks, to return home safely and confidently.
Sadly, convalescent homes have now almost disappeared and the consequence is the increasing trend for hospital re-admissions with national statistics showing that, especially where older patients are concerned, the re-admission figures are high with many having to return to hospital within a month.
